Patient Demographics
You may have to add or make changes to demographics on a regular basis.
A good rule when working with patient demographic information is to fill out absolutely everything you can. Navigate to Patients | Registration / Billing and complete all the tabs that apply to your practice.
Information that you enter is automatically saved when you move off the field or tab.
Registration / Billing Tabs
- Patient Information
- Contact Information
- Consent Information
- Insurance Information
- Responsible Party
- Financial Notes
If you are unsure where specific information is located, see Patient Demographics - Where do I find
Edit Locked Fields

You must have access rights to change these because you could easily create a duplicate or change important identifying information. These fields are:
- First Name
- Middle Name
- Last Name
- Former Name
- Date of Birth
- Date of Death
- Social Security Number
Some General Information about Patient Demographics
Phone Numbers
Phone numbers have a blank field to the left. This is for international calling codes where necessary.
Zip code fields
Include 2 parts - the 5-digit zip and the plus-four code on the right.
Where you see the binoculars button to the right of a zip code field, you may enter the zip code and the city and state will pull in automatically.
Copy Buttons
Where you see the copy button, you may copy the patient's Present Address into the new address fields (e.g. Permanent Address, Responsible Party, or Emergency Contact)
Merging Duplicate Patients
If you discover a duplicate patient, you will need to merge the 2 records. For additional information, check out Merging Duplicate Patients.
Deleting Patients
The rule of thumb is to NEVER delete a patient record. For this reason, most users do not have access to delete patient or undelete a patient. Please coordinate this with your office administrator.
